Lax4Lives 2009
Lax4Lives is partnering with the HEADstrong Foundation to present the Fifth Annual Lax4Lives Charity Tournament. The event will be held Tuesday December 29, 2009 at the Sportsplex in Feasterville, PA. The tournament is open to boys’ high school, travel and club teams. Profits from the tournament will go to support the efforts of the HEADstrong Foundation. The cost per team is $600, all checks made payable to The HEADstrong Foundation.

- High school boys teams
- Three 25-minute games, plus playoffs
- 5 v. 5 plus a goalie (no long poles)
- Morning and afternoon sessions
- Each team plays in one session or the other

College and Masters Games
The annual Lax4Lives post tournament games will start at 7:00 PM on Tuesday December 29th. The College player game will take place on Field 2 and the Masters game will be played on Field 3. A $10 minimum donation per player is requested.
This will be followed by a post-game celebration at Kenny's Spirited Eatery. Mention Lax4Lives and 10% of your check will be donated to Lax4Lives.

Contact Us and provide the following information:
- Team Contact Name and Email Address
- Preferred Session (AM or PM)
- Complete the Team Roster Form
- Each player must complete the Player Waiver Form
- Bring the Team Roster, Player Waiver forms and $600 registration fee (checks payable to The HEADstrong Foundation) the day of the tourney.
